The spatula can be described as "Riposizionatore iris"

Its utility is obvious because, on the contrary of the spatulas or hooks for repositioning of the IOL, normally used in the case of prolapse of the iris from the tunnel or hypo / atalamia in correspondence of the tunnel, this spatula flattens the entire iris engaged simultaneously, facilitating the leakage of viscoelastic; in fact spatulas used for repositioning the iris, being much thinner, crush the iris prolapsed into a point, facilitating the further prolapse of tissue adjacent the iris.

In the video there isn t a true prolapse of the iris, because I used the trowel immediately before it happened, but the mechanism of action sees well enough
